Merchant Surcharge Disclosure Form

At this time, Mastercard is presently no longer requiring that U.S. merchants register their intent to surcharge credit cards in the U.S. on this site.  Mastercard is in the process of updating its rules around surcharging, including any merchant or acquirer notification and registration requirements, and anticipates announcing those revised rules sometime in the 2H of this year.  Pending the announcement of those revised rules, Mastercard will not require merchants register their intent to surcharge.
